New Patients

To register, a patient will be required to fill out a simple registration form giving details such as their name, mobile number, address and their NHS number. To enable the medical staff to trace the patients previous medical records they will need to know the patient’s previous Practice address. Failure to fill in the details can lead to a delay in the registration process . Please also make a note of your email Id , if you have one . This will be used for the surgery to be able to contact you for reviews and appointments

The Practice Leaflet is available in English and Welsh and a copy can be collected from the reception desk. A larger print leaflet is available on request.
